Petition Letter: Dear Dr. Johanne Bénard,
I am writing to appeal the maximum of 6.0 units transferred on the Letter of Permission that I have already been granted. If possible, I would like to transfer a maximum of 18 units for the Fall and another 18 units for the Winter term at Carleton University. I would essentially like to complete my fourth year core courses and electives at Carleton.
As stated on my initial appeal for a LOP, I have not been in school since mid-November, 2016 in order to be at home in Ottawa. I made the decision to leave school for the current academic year because the state of my mental health made it too difficult to complete my studies. Normally, I am more than capable of completing my assignments and have passed almost all of my courses in good academic standing since first year. Most of my days during the two and a half months at school consisted of poor appetite, very little sleep and very low moods. Furthermore, I have always had a hard time adjusting to life in Kingston and found that to be a significant stressor on my body and mind during that time. I would go to Ottawa every weekend to relieve myself of the pressures and stress of school. My personal problems began to significantly affect my studies when I was assigned a short 750-word film review by one of my favourite professors, Dr. Petra Fachinger, in one of my most enjoyable courses, ENGL482, Indigenous Literatures. This is when I began to feel alarmed about my inability to exert any control on my academics even for classes that are extremely important to me. It took me almost two months past the deadline to finish. The latter half of this assignments completion was done while I was in Ottawa and still proved to be a struggle.
In the midst of these obstacles, I decided to meet with Sarah Horsford, the faculty of Education's counsellor, and Spring Forsberg, the academic advisor for Arts and Science students. Since then, I have seen my doctor on multiple occasions, been diagnosed by a psychiatrist and recently started therapy.
Given that I am in therapy, I have developed a relationship with my therapist in Ottawa and require her guidance to recover. Furthermore, I am still susceptible to relapse and I have been advised to stay home in Ottawa. I am driven to complete my studies, but I believe that it is in my best interest to finish my degree at home in order to also prioritize my mental health.
Attached to this application is my psychiatric assessment as a supporting document in order to validate my need for this appeal.

Academic Plan
Academic Plan: I plan on completing my Concurrent Education degree by the end of the 2018-2019 academic year with the grades that satisfy my expectations. I do not think, nor does my doctor, that I can achieve this by going back to Queen's for the academic year of 2017-2018. It is much too soon and I need to be close to my doctor, my family and other sources of support. Additionally, as stated in my appeal letter, living in Kingston has only contributed negatively to my circumstances.
I will be applying for OSAP soon as I would like to attend Carleton starting September 2017 and I will likely take four courses per term. Leftover courses will be done in the summer of 2018. I have taken a look at the courses I am expected to complete at Queen's and found similar ones at Carleton. I have already been approved for a Letter of Permission and intend on committing to my final year at Carleton. I have already chosen my courses and I am in contact with the department. I am aware of all the steps necessary in order to be approved for these courses at Carleton. While completing my fourth year in Ottawa, I will also continue taking my medication and attending psychotherapy as it is extremely important for my recovery.
