Everything in the newly uploaded doctor's note refers to my diagnosis, academic struggles when I initially got diagnosed in November 2016 (i.e. my doctor references me subsequently dropping calculus in the same timeframe as my diagnosis), symptoms throughout the academic year in which ADHD directly affected my academic performance, and my prognosis.
