Petition Letter: Dear Associate Dean,
I am appealing to drop BIOL 102 (Fall, 2016) and to receive a CR standing for PHYS 104B (Winter, 2017).
The reason I am asking for an appeal is because of extenuating circumstances due to a health issue. Due to this I faced several symptoms that severely affected my academic capabilities. The symptoms persisted throughout the second half of the fall term and throughout the entire winter term. The symptoms persisted even after winter term hence the reason for the late appeal. The supporting documents provided are verification of illness forms that were provided by the doctor I was seeing in Queens Student Health. The forms provided state the severity of the condition and its effect on my academic ability as well as it gives a timeline. To deal with the issue I was seeing both a family doctor at the wellness center and a specialist at Hotel Die Hospital during this time. I am currently seeing another doctor here at my home city to continue treatment. For BIOL 102 I was incapable of doing the final exam due to the circumstances, so I contacted the instructor for a later exam date during the second term as I was hoping I would recover by then. Unfortunately, the condition never improved as I hoped so I didn¿¿t get to write the final exam. This is also the reason I was unable to submit an appeal for BIOL 102 during the winter term. For PHYS 104 I am asking for a CR standing because of the same reason. I finished the course with a B- as my grade which I feel was lower than what I could have gotten in the course had I not been dealing with an illness that heavily impacted my academic ability. My mid-year grade for PHYS 104A was a 90 so my overall grade had deteriorated over second term due to the illness.
For my future plans for academic success I plan on continuing to have regular meetings with medical professionals to help find out a better treatment plan for me to ensure I can become academically functional again. Once I make some form of recovery I do plan on retaking the course I dropped and attempt to get back on track for my degree.
