Submit an Academic Petition or Appeal

Petition Letter: I am requesting an extension for my incomplete grade in Abnormal Psychology, PSYCH 235.
I had extenuating circumstances, beyond your control including physical illness, a psychological challenge and family problems.
I had academic accommodations and had requested my exam not be the day after my other.
By the time my professor and her contact got back to me (this was not without me contacting her again because I had not heard back) I was days from having abdominal surgery. The professor then deferred my exam. It took numerous weeks to heal from my surgery to be able to sit for at least a 3 hour period. It was at this time that I had to be weaned off a psychological medication, go without anything for a week and then start taking a different one that took weeks to start working. In this time period which it is still not working to this day and was doubled in dosage today. This was not and has not been a new experience for me it has been going on for at least a couple of years. I was completely up front, honest and kept in close contact with my professor the entire course time having a few meetings with her to discuss academic help while I struggled with these issues.
I am asking to PLEASE be able to write this exam.
