Dear Dr. Atkinson and Dr. Bénard,

Thank you for granting my request to waive the one-term deadline for the submission of my appeal.

I am appealing to drop following courses: CISC 453 (Winter 2016), COGS 400 (Fall 2016), MATH 111 (Fall 2016/Winter 2017), MATH 121 (Fall 2016/Winter 2017).

As mentioned in my deadline appeal and elaborated upon in the attached professional documentation, I struggled during the period being appealed with mental health issues that severely impacted my academic performance. This adversely affected my lecture attendence, my coursework, and my studying, despite my great personal interest in the course material. Ultimately this led to my failure of all of these courses, which limited my ability to proceed in my chosen degree (an effect which I unfortunately did not fully recognize until discussions with academic advisors much later).

Therapy has been ongoing throughout, and I am continuing to pursue psychiatric support. This has been a great resource in my recovery, and I have been very fortunate that Queen's has provided me with such an effective system of mental health support. Accordingly, my psychiatrists account of the issue and my current state (both for this appeal and from a similiar appeal from a prior time period) is attached, as it was with the initial deadline-waive appeal. I believe that this - as well as other personal and interpersonal developments that I've since made in my life - have positioned me well to make the best use of the opportunity that this appeal provides, and complete the degree that I set out to achieve before mental illness intervened.

I'd like to thank you again for your incredible generosity and patience in considering this appeal; the chance to pursue this accomodation has been greatly appreciated, regardless of the outcome.
