Petition Letter

Petition Letter: To Associate Dean (Studies),

I am writing to request to have GRMN 101, HIST 269 from the Fall 2017 semester, ARTH 212 from the Winter 2017 semester, ENGL 100, HIST 250, HIST 312, from the Fall 2016/Winter 2017 semester and HIST 303 from the Fall 2017/Winter 2018 semester, HIST 215, HIST 257 from the current Winter 2018 semester dropped from my transcript.
I have been experiencing some mental health issues that have affected my performance in my classes. It has been keeping me from attending classes, doing assignments and readings negatively affected my sleeping schedule and eating habits. These issues began to arise right before the winter break in December 2016 and slowly became worse as the semester went on and I began to fall behind and eventually fail my courses. I decided to get help from the counseling services in starting April 2017 and then again in October 2017, continuing on to the current time. Then, I was referred to a doctor to improve any physical issues that may have been affecting my health. I was prescribed a couple medications which I am still taking. I did not decide to drop the courses by the drop date because I wanted to try to catch up in all of my course work; however, I was still too affected by my mental health.
To improve my academic performance, I will continue to seek help from the on-campus doctors and counselors and take a reduced course load. I will also take steps to solidify a routine to fix my sleeping schedule and eating habits which will help my mental health.

Sincerely,
Courteney Lew.
