Petition Letter: Dear Associate Dean of Arts and Science,

I am writing to you, kindly, for an appeal to waive the 21-day appeal deadline to drop courses and to allow me to drop math ECON261 (F2006), MATH 126 (F2006/W2007), MATH 121 (F2007/W2008), CHEM 112 (F2007/W2008), and STAT 263 (W2008) due to extenuating circumstances.

I spoke with Dr. Vetere at Queen¿¿s Learning Disability Services in the Spring of 2013 to begin the appeal process, but unfortunately Dr. Vetere was absent due to health concerns to continue the process. I re-took some courses in order to increase my GPA, but I was unable to achieve a higher grade. In February of 2014, I met with Mrs. Spring Forsberg in regards to my academic status and I asked her if I could still request to drop ECON261 (F2006), MATH 126 (F2006/W2007), MATH 121 (F2007/W2008), CHEM 112 (F2007/W2008), and STAT 263 (W2008). Mrs. Forsberg did not indicate an outcome, but she encouraged me to apply for the appeal and provided me with steps to 1) request to waive the 21-day appeal deadline 2) and to request to drop the required courses past the final drop deadline.

Dear Associate Dean, I am kindly requesting you to waive the 21-day appeal deadline because there are five courses where I acquired failures, but I did not have midterm or exam accommodations. I was formally diagnosed with a learning disability ADHD and anxiety at the end of the winter term in 2009 by Regional Assessment and Resource Centre (RARC), in which they recommended accommodations, medications, and other information to help me during school. With RARC¿¿s, I was able to sign up at Queen¿¿s Health Counseling and Disability Services and commence to receive exam accommodations and other supportive techniques, I was able to get medical prescriptions for ADHD, and I began to understand my learning disability.

If my request to waive the 21-day appeal deadline is granted, I am kindly appealing to drop the following courses after the drop deadlines due to extenuating circumstances presented above, which consists of my learning disability-ADHD and anxiety, and not having exam accommodations or having access to medications for ADHD; the courses are: ECON261 (F2006), MATH 126 (F2006/W2007), MATH 121 (F2007/W2008), CHEM 112 (F2007/W2008), and STAT 263 (W2008).

Please find my report from the Regional Assessment and Resource Centre (RARC) in my files(or attached) that will support my appeals to 1) waive the 21-day appeal deadline because I did not have accommodations for these exams and 2) to request the dropping courses after the drop deadline for the following courses: ECON261 (F2006), MATH 126 (F2006/W2007), MATH 121 (F2007/W2008), CHEM 112 (F2007/W2008), and STAT 263 (W2008).
