I am submitting this request to appeal the Faculty's decision on denying a change of grade for my fall course CISC 365 (Fall, 2017). Missing the deadline to submit this form was out of my control and can be verified by Dr. Robin Dawes. -------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------------
Due to a severe car accident that my mother was involved in at the beginning of November of 2017 I was unable to attend my classes for the remaining time of the fall term, as she needed 24-hour home care. Dr. Robin Dawes was very kind in offering to submit an Incomplete grade for me at the end of the term and later, once I'm back in school, compile a single test to cover the material that I missed. During the following winter term, my mother's condition allowed me to continue on with my studies. I continued to visit my mother in the weekends to (as her doctors asked) monitor any life-threatening changes in her condition over time, ones that she herself wouldn't notice.

My weekend visits and classes during the week kept me quite busy until the end of March, at which point I contacted my professor. I didn't receive a reply to my email and I tried again around mid-May. Shortly after this, I noticed that my grade appears as an F instead of IN on Solus, which was somewhat worrisome. Still unable to reach my professor and as recommended by another professor in the Computing Department, I wrote another email, printed it out and slid it underneath his door. I dropped it off at his office hoping that a physical copy of my letter would assist me in reaching him as soon as possible.

Dr. Dawes emailed me shortly after this and informed me that I will be writing my test at the end of September and in the mean time he can submit a change of grade request for me. I am hoping that the appeals procedure regarding this request will be resolved shortly as my current grade for the course is interfering with my ability to register for all of the required upper year courses of SODE.
