To the Dean and Committees concerning Undergraduate Education:
I am petitioning for a retroactive withdrawal from the fall semester 2018 for medical reasons. In the third week of fall, I was deeply suffering from diagnosed Bipolar Depression.
When these incidents come up I feel nearly manic and that I can do it ALL. Unfortunately because I haven't taken a formal Math course since 1998. I thought I could really try hard & make it through the course. Although I was able to keep up with my courses at first, sometimes I was too overwhelm from life to push through the class and I fell behind in my work. Although I was behind, I felt I could keep making up the work. My poor 12 year old daughter has taken a lot of my blame and she doesn't need this at all. This is a child who had to be hospitalized for suicidal tendencies and cutting behaviors over the past summer vacation.
I was hospitalized during the summer months to boot as I crushed my car into a hydro pole forever damaging my two youngest children. My right eye is damaged now for the rest of my life. I have never been so bleak I am ready to give up. I realized then that I was just not going to be able to make up the work in MATH I had missed and tired of looking like a failure in my kids and my own eyes. I am already past the withdrawal deadline at this point.
I would appreciate it if YOU the DEAN and the Faculty Senate would allow me to withdraw from this MATH COURSE as I need HEAVY TUTORING BEFORE ANY MORE MATH WILL BE APPLIED FOR at the university.
I have contacted Sarah McLean Knapp and is she more than happy to take me into the course.I will work hard to show you I am a great student. (Digital Media which actually works for the ECO Course as we as living in an age of digital EVERYTHING

Academic Plan: My Appeal Plan is simple which is ideal for me to continue to excel.
Ask for help from peers, TAS, Profs when stuck in a course (this is a social phobia I am working on now)
Talk to QSAS if need any MORE accommodations or recommendations
GET ON CAMPUS I am so sad because I cannot until my youngest child reaches JK next year. Than I have SOOOOOOO much better assistance through the school - stress managment, libraries like you wouldn't believe and a sense of belonging. not to mention real life study groups to attend as well.
ACACEMDICALLY I FINALLY GET TO TAKE THE DEVS course I have been wanting to take for the past 2 years.
There will be no more hard courses with hard courses. One easier with one hard is what this student and mother (and her family can handle.)
I need the ARTF course granted or I have no means to feed 3 kids nor keep them warm in the Winter. (OSAP NEEDS A 40 for perm. disabled students @ full time status.) With the math drop i went to pt status.
I made a mistake. Please assistant me in this process. (ALL PREVIOUS DIMPLMOAS I have completed I gradeuated the top of the class.... I can do this.)
