Submit an Academic Petition or Appeal

Petition Letter: Dear Sir/Madam,
September 23, 2018

I received an e-mail from Dr.Christopher Ward regarding the departure of academic integrity for a disease assignment . I have not plagiarized the research assignment and it¿¿s my own work . It¿¿s a research assignment and it was mentioned on the assignment guidelines to do search for the disease and write in your own words . . I followed the instructions and did the same . I have used the research articles to get information to write the report .
I took this course last year and I dropped me because I was not given permission to write the exam . I assume that instructor doesn¿¿t like me because I have seen that previously I was not given chance to write the exam even though I had legitimate reasons and documents . My mother was sick and I had to take care of her . I was not given a chance to write the exam and this, affected my grades and also affected ne financially. I have been not given OSAP because I was an interest student and I paid tuition from my mother¿¿s account . My mother is a single mother of three and she has diabetes . There was a domestic violence in our family that lead to the separation of my parents . I had suffered violence and family problems which put me into mental health problem . I have mental disability and now, I am in major depression since I received an e-mail from PHGY216 instructor regarding departure from academic integrity . I always feel like I am guilty and everyone is bothering me without any reason . This assignment worths 25 of my grade and it will affect my grade badly. I request you to please give me mark or please give me permission to write it again .I achieved B+ grade in PHGY215 Course . Please give me an oppurtunity to improve my grade . Please give me permission to rewrite the assignment or add my 25 to other assignments in the course . Please don¿¿t take my 25 marks off. This will give me a big panic shock .
