Petition Letter: 1. The Requests
I am appealing five courses that I was not able to complete during the winter term of 2018. In April 2018, I was granted incomplete standings in the five courses until September 15th, 2018. Though, I was not able to complete the five courses by September 15th, 2018 for reasons outlined in section 2. of this letter. The specific requests are as follows.
To drop CHEM 112 (Fall/Winter, 2017/2018) DR
To drop BIOL 103 (Winter, 2018) DR
To request an aegrotat standing in PHYS 117 (Fall/Winter, 2017/2018) AG
To request an aegrotat standing in MATH 121 (Fall/Winter, 2017/2018) AG
i. To receive an extended incomplete in MATH 121 (F/W, 17/18) if request 4 is not granted IN
To receive an extended incomplete in PSYC 100 (F/W, 17/18) if request 5 is not granted IN
2. Explanation of Extenuating Circumstances
In April 2018, I experienced an exacerbation of ongoing conditions that prevented me from completing the five courses mentioned above. For that extenuating circumstance, in April 2018, I was granted incomplete standings in the five courses until September 15th, 2018. I was expected to receive treatment and rehabilitation services from May 2018 to September 2018, and to continue rehabilitation and management services into the 2018/2019 school year. However, I did not receive treatment or rehabilitation services from May 2018 to September 2018 for reasons outside my control. Instead, I received interim support until I was able to begin treatment. I began treatment in late October 2018.

3. Academic Plan
The following paragraph includes an overview of my rationale of the requests outlined in section 1. and an overview of my academic plan following this appeal process.
In requesting to drop CHEM 112 I plan to complete CHEM 112 or equivalent by 2020. In requesting to drop BIOL 103 I plan to retake and complete BIOL 103 in winter 2019. In requesting an extended incomplete in PSYC 100 I plan to complete PSYC 100 2017/2018 by writing the PSYC 100 exam by the end of April 2019. As I have completed at least sixty percent of the course work in MATH 121 I am requesting to receive an aegrotat standing in MATH 121 2017/2018. As I have completed sixty percent of the course work in PHYS 117 I am requesting to receive an aegrotat standing in PHYS 117 2017/2018.
