Petition Letter: Dear Associate Dean (Studies),
I am appealing to drop PHIL 259 (Fall, 2018) due to extenuating circumstances starting November 8 2018. The extenuating circumstances had serious impairment in academic functioning and persisted past the Fall term. Supporting documentation include the Verification of Personal Health Condition and documentation from Queen¿¿s University Student Wellness Services (incoming).
A physical health condition temporarily limited both my academic participation and ability to meet academic requirements. The date of onset of impairment was recorded on November 8 2018 and persisted past the Fall term. The condition had serious impairment in academic functioning including unable to attend class, submit assignments and attending an exam. In particular, my performance in PHIL 259 was substantially affected and resulted in a final grade of F; after scoring F in two of three assignments and missing the final exam. As a full-time student, I made the decision to focus the remainder of my limited abilities on the remainder of the courses in an attempt to salvage what I believed was possible amid ongoing extenuating circumstances. The decision to reallocate my focus was received with support from both of my health care provider Dr. Robinson and counsellor Ms. Tahany Okby. Despite the final grade of F received in PHIL 259, the support from Dr. Robinson and Ms. Okby made it easier to cope, and Spring Forsberg-Lewis from Student Services at the Faculty of Arts and Science helped me with the Appeal process to drop PHIL 259.
I plan to regulate my physical illness by regularly consulting with my health care provider and seeking the services of Queen¿¿s Student Accessibility Services. This plan will allow me to continue as a full-time student with the ability to achieve academic success for the Winter 2019 term and onward.
