Attention,

I am currently enrolled in second year biology at Queens. I have previously been diagnosed with a generalized anxiety disorder and depression; throughout my University career at Queens I have struggled with anxiety and depression. Over the last three years it has caused me to have to Late Drop a number of courses, you will see this in my records.

This past term (Fall 2018) was especially difficult for me with my depression debilitating me at times. I found it nearly impossible to get out of bed, focus or attend my classes throughout the term causing me to miss a large portion of the lecture and tutorial materials. I found myself unable to catch up come exam time and spoke to my professors before my final exams about deferring my exams (in the three courses I did not drop) and they allowed me to delay writing my exams until after the Christmas break. I was able to write two of the three exams on the first week back in January and had the BIOL 205 exam scheduled for the 14th of January. Although I did try very hard, I was unable to sufficiently prepare for the BIOL 205 exam due to the monumental amount of work that I had to catch up on. I am asking that I be allowed to late drop this course in order to avoid a fail on my transcript. I am working daily to help my mental health and it is an ongoing effort. I appreciate Queen¿¿s support with this.

I have attached a note from my family doctor, explaining the circumstances further.
