Dear Associate Dean (Studies),
I am appealing to drop CHEM 112 (Fall 2018 to Winter 2019). I have struggled with this course and despite a full year effort I am appealing to drop this course. It is a required course for the degree plan I had chose and will no longer be pursuing.
I have been experiencing psychological challenges and family problems for the entire duration of my undergraduate degree. Since my mother's passing in July 2016, I have been without parental support which has impacted my functioning. Family problems occurred around December 2018, and are still present, which has greatly impacted my performance. In addition, I have been experiencing medication changes since Fall 2018.
I have been regularly meeting with a physician, psychiatrist, and counsellor from Student Wellness Services, and I am registered with QSAS. I am currently on a waiting list to begin psychotherapy with another health care professional.
My plan for the future is to switch to a degree plan that is more suited for my academic background and strengths and to continue with regular appointments with health-care professionals.
Thank you for your consideration.
Sincerely,
Jayde MacMillan
