Petition Letter

Petition Letter: Dear Sir/Madam,

I am Jingtao Chu, a 1st year student of the Bachelor of Music Program at the Faculty of Arts and Science. I received from the faculty a letter on May 21, 2019 which required me to withdraw for one year from the Faculty. On the ground that my recent academic performance has been significantly affected and my ability of on-going study is being seriously impacted by a kind of extenuating circumstance which is beyond my control so far, I¿¿m appealing to:

1. Have the above-mentioned Requirement to Withdraw waived.
2. Late-drop all the courses I enrolled in, MUSIC 111, MUSIC 121, MUSIC 127, MUSIC 129, and
MUTH 191.
3. Be granted a Voluntary Withdrawal from the Faculty till September 2020.

I registered as a 1st year student at the beginning of the Fall Term 2018 and I did quite well in that term. However, from the beginning of the Winter Term 2019, some kind of mental health issues, with which I had been struggling for a while, began increasingly impacting my ability to deal with the pressure from the study load and seriously hindering my ability to go outside to attend my classes, even leading to a lot of absence from my classes.

I sought and received help from the Student Wellness Centre and even went to the Emergency Department of KGH for emergent help once, but the issues are still afflicting me now. I was referred by Rina Gupta, who is the director of the Counselling Services at the Student Wellness Centre, to the Psychology Clinic at Queen¿¿s University to do a thorough mental health assessment in March 2019. The assessment lasted about 3 months from March 21 and the final report was released on May 13, 2019. According to the report, I am currently suffering a kind of mental health illness and I was recommended that I should take at least a one-year break from school to get better treatment. I¿¿ve now begun a treatment program at the Psychology Clinic at Queen¿¿s University.

Above is the reason why I submit this appeal. I wish I could recover from the illness and get back to school before the beginning of the Fall Term 2020, retaking all the necessary courses to continue my study at Queen¿¿s University.

A supporting document will be provided by Rina Gupta separately. More detailed health information, if needed, will be available on request.

I sincerely wish my appeal could be approved.

Thank you for your attention and your kind understanding.

Sincerely,

Jingtao Chu

Academic Plan

Academic Plan: As per my current situation, it's hard for me to work out a detailed academic plan so far. My expectation is that I could get a good treatment during the one year break and could get back to school at the beginning of the Fall Term 2020, retaking all the necessary courses to complete the Bacelor of Music Program at the Faculty of Arts and Science. I will take all possible opportunities to sharpen my guitar performance skill and to improve other related knowledge and skills during the break.
