I've taken this paper to the notary office to translate, but it will take at least three weeks for them to check with the Hospital and then do the translate, so I did the translation myself and I have the original form in Chinese is also down here.
Translation of the Doctor Remarks: Patient Hongbo Liu, Chinese ID number 110111199908017015. Right shoulder dislocate around May 2017 for the first time by injury, and after then dislocation took place several times, the latest was during March 2018. After the dislocation, the patient can not do exercise normally, seriously influences daily life and mission of study, and the patient has been mentally anxious, has taken the Arthroscopic Bankart Repair surgery around June 2018, hereby certify.

Hongbo Liu got injury around May 2017 in a rugby game.
And has dislocation four times during the year after the injury.
In his first year of university, he has been in an anxious state. He was not willing to head outside to activities and not willing to make connections with other people, not willing to get help.
In the summer of 2018, he came to psychological consultation.
Asses that the client has been in an anxious mental state, and suggested to take a ten times short term psychotherapy. But the client believes his unhealthy emotion has been released and would be able to spontaneous remission.
Starts to take systematic psychotherapy in May 2019
The client has mentioned that in his second year in University he did not get rid of his psychological block. Has to find himself dragged down with Procrastination, not facing the actual problem in life.
Till now the psychotherapy has started for one hand half month, Hongbo Liu has a good effect in the psychotherapy, inner emotions being better and better and starts to restore confidence, management events by good planning.
