Submit an Academic Petition or Appeal

New Information: With this new information I will be attaching pictures of the medication bottle I was prescribed. I have another piece of medical documentation that I would like to submit (a doctors note) but the physician who wrote it for me no longer works at that specific clinic anymore, and since the note is dated back to 2018, the clinic is not allowed to release such outdated information to me. The clinic is also in my hometown (6 hours away) and I'm not sure when I will be going back to retain a re-written note. I've communicated to the clinic that I need this medical note for an appeal process for university but they still won't release that information. The clinic communicated to me that they are more than happy to speak on this situation; their number is 519 691 0100.
