Petition Letter

Petition Letter: To Associate Dean (Studies),

I am writing to request to have GRMN 101, HIST 269 from the Fall 2017 semester, ARTH 212, ENGL 100, HIST 250, HIST 312, from the Winter 2017 semester, JAPN 200 from both the Winter 2017 and 2018 semesters and ASTR 100 and ENGL 100 from the Summer 2018 semester dropped from my transcript.
I have been experiencing some mental health issues that had affected my performance in my classes. It had been keeping me from attending classes, doing assignments and readings negatively affected my sleeping schedule and eating habits. These issues began to arise right before the winter break in December 2016 and slowly became worse as the semester went on and I began to fall behind and eventually fail my courses. I decided to get help from the counseling services in starting April 2017 and then again in October 2017, continuing until January 2019. Then, I was referred to a doctor to improve any physical issues that may have been affecting my health. I was prescribed a couple medications which I am still taking. I did not decide to drop the courses by the drop date because I wanted to try to catch up in all of my course work; however, I was still too affected by my mental health.
To improve my academic performance, I have decided to take at least a year off from school. During that time I will continue to seek the aid from a psychiatrist and work on strategies that will help me to manage my work. Also, when I return to school, I decided I will take it slowly and only attend school part-time to make the re-entry as smooth as possible.
