Petition Letter: To whomever it may concern,
I am writing this appeal letter in the hopes of being able to drop the two courses I am enrolled in: BIOL 102, and MATH 121A. I understand that it is past the date to drop a fall term course. My extenuating circumstances are related to mental health. I have been dealing with depression for a few years, and I have noticed (especially recently) that it is having a large impact on my academic performance (ie. bed-ridden and cannot make it to class, concentration/motivation issues, increased suicidal ideation due to academic stress).
A few days before the last official date to drop classes, I dropped three classes (PSYC 100, CHEM 112 PHYS 117), hoping that having a lighter course load would be easier for me and would not affect my mental health as much. (I would be making up for the dropped courses during the summer). When I spoke to the academic advisor before dropping these classes, she told me that I could drop all my classes and return home, but I wanted to stay at school for a bit longer and try again.
Over the past few weeks, I have found that I have been severely affected by my mental health and am struggling to function and take care of myself daily. I speak to an online counsellor that I found through the university, called Empower Me. As well, I attempted to make an appointment with a physician through the school for a mental health assessment, but was unable to make the appointment because all of the spots had been booked. So, I went to the emergency department at the hospital and spoke to a nurse, social worker, and physician who agreed that it would be best for my mental health if I returned home.
The reason I am not appealing to defer the exam is because I wish to return home and recover from my mental illness in a stress-free environment. I would rather not have to deal with the stress that comes with school in general, and would like to take a break from school while I recover.
As for supporting documentation, the physician at the hospital has signed an official Kingston General Hospital emergency department document which states that I am seeking a note for extenuating circumstances (ie. mental health).
On the note of achieving future academic goals, I hope to perhaps return to Queens or another institution to complete my post-secondary education at a later time. I plan on attending an inpatient program at a psychiatric hospital before focusing on school again.
