Submit an Academic Petition or Appeal

Petition Letter: Dear Deans:
I am a 3rd year BSH student in Queen¿¿s University. I am writing this to appeal for a Credit Standing in the course Math280. I took it in Fall 2018, it is late to submit this appeal letter, but I really have some uncontrollable reasons, I will state them in this letter.
During the summer of 2018, my grandma was diagnosed as cancer. In earlier September, just after I arrived Canada for my 2nd year study, I heard the worst news from my home country: She was already in the terminal stage of Cholangiocarcinoma(a kind of cancer), which means she came to the end part of the life. For the rest of the term, I almost heard from families every week that my grandma's health status get worse and worse. My grandma lived with me since I was born, I spend most of my previous life with her since my parents were both busy with work in another city during my childhood. I really did not know how to face the truth that she will leave me anytime, and I may not be able to see her anymore.
She chose to spend the last several months at home, therefore, I thought there was not any diagnose record from September to December, which can prove the authenticity of this academic appeal. As a result, I gave up to submit this last term and passed the deadline. However, when my families were moving home last week, a diagnose record on 11th September was found. It state her bad physical condition. I will put that in the document page. The date is also on it, hope it can make this appeal much more persuasive.
Above is the main factor which seriously impact my mental and physical status. At the beginning, I suffered from sleeplessness. One month later, I became anorexic. I suffered a nervous breakdown for the rest of the term. Because of this continuous depression, my health was impacted. I experienced irregular menstrual period for 3 months. I went to the Walk-in Clinic in Lasalle Building for medication on October 26th, the electronic prescription by doctor was directly sent to pharmacy in ARC. I cannot provide the prescription document at this time, but I took photos in consulting room that day in order to let my mother know, as she was concerned with my health. I will upload the photo in the document page, the date shown by my cell phone will be included. I will also ask if it is possible to inquire for my earlier prescription in their system later.
I was planning to go to Student Wellness Service to find psychotherapy for my depression, however, I am an international student and English is my second language, it was difficult to express my complex feeling clearly. I have talked to another doctor about my painful experience, unfortunately, he did not provide me useful suggestions.
For both physical and mental reasons, I felt so hard to focus on my work. I tried my best and did not get too bad results in other 4 courses, but I only got a D+ in Math280. The main factor of this is that I got a low grade in November midterm, since I was really in a bad time in November.
After the final exams, I went back to China(I will also attach the flight information sent by Air Canada), that was the last time I saw my grandma alive. She passed away in Feb 2019. I had a long talk with my parents at home. They encouraged me and finally realized I cannot keep this bad status. After I came back to school for 2019 winter term, I worked very hard to make up my GPA, since my goal is to enter in the graduate program in Queens, which requires an excellent academic standing.
I never got grade lower than B in other courses, and mostly above A. I was on the dean's honor list for both 1st and 2nd years. I will also put my unofficial transcript in the document page, hoping it is easier to have an overview of my academic performance.
Overall, the low grade in Math280 was unexpected which was affected by my unhealthy physical status, it did not reflect my true academic level well. Therefore, I apply for a Credit standing in this course. I sincerely hope you can give me this opportunity to repair that period of bad time.
