Petition Letter: Hello.
My name is John Douglas Breault.
Student Number: 10056217 in Arts & Science.
I'm requesting that the courses that I am being charged for during the 2018-2019 school year be removed from my record and dropped without payment, along with any charges accumulated during this period.
The reason for this request is that I did not attend a single day of school during this term, nor did I hand in any assignments or complete any examinations.
The reason for this absence is due to a number of mental and physical health issues that I've been struggling with for some time.
These issues rendered me unable to properly attend to the matter until now, for which I deeply apologize for the inconvenience.
On a side note, during this period a member of my extended family received a message from a representative at Queens informing them of the need to attend to this matter before it goes to a collection agency, which it has since done.
This family member, however, never passed on this information due to some family issues and I hadn't even been aware of the fact until the collection agency called me.
Again, I apologize for the inconvenience, but I simply wasn't able to begin resolving this until now.
Attached to my appeal form is the necessary doctor's documentation, supporting my claims and asking as well that this be resolved in the manner stated above.
I hope I've addressed all the details necessary to bring this matter to a conclusion.
