Submit an Academic Petition or Appeal

Petition Letter: I originally attended Queen¿¿s University during the fall of 2004, to the winter of 2006, intending to major in psychology. I ended my studies due to deteriorating mental health issues. At that time I was under the care of Dr. Lorraine Philp, please see the attached letter. I became determined to finish my studies and enrolled at Queen's University online from the fall of 2012 to the winter of 2013. I am requesting to have the appeal deadline waived. If the appeal deadline is waived, I will ask to have the final grades of the following courses to be removed from my transcript due to the fact that circumstances out of my control made it impossible to complete these courses:

¿¿¿¿¿¿¿FILM 240 (fall, 2012), HIST 125A (fall, 2012), SOCY 122A (fall, 2012), SOCY 122B (winter, 2013), and HIST 125B (winter, 2013).

I am appealing to waive the deadline because that event triggered and exacerbated an ongoing history of severe mental health and addiction issues and these circumstances¿were ongoing from 2012 to the present date and prevented me from initiating the appeal at that time. In November of 2012, my boyfriend of three years with whom I was living with hung himself because I ended the relationship, please see the attached reports from my psychiatrist at the time, Dr. De Souza, MD. This event resulted in a complete mental breakdown. I could not function, let alone continue my studies, and was subsequently heavily medicated. Since then I have been diagnosed with Major Depressive Disorder, Generalized Anxiety Disorder, PTSD, panic attacks and Borderline Personality Disorder, please see the attached letter from my current psychiatrist, Dr. Michael Shaytzag, MD. I have been admitted to the inpatient psychiatric unit at Oakville Trafalgar Memorial Hospital multiple times for suicidal ideation, please see the attached emergency room reports. I have struggled with major drug and addiction issues since that event, and continue to struggle with these issues up to the present day. Iam on ODSP for mental health issues and cannot financially support myself, please see the attached letter from my ODSP worker. I have only recently gotten my life together to start working on my future - I want to go back to school to become a social worker and help people who are suffering, drawing from my own experience.¿
