Submit an Academic Petition or Appeal

Petition Letter: Dear Associate Dean,
I am appealing to drop CHEM 112 A/B from the fall 2019/winter 2020 semester because I feel I was not mentally capable of completing the course. Due to the pandemic my mental health has suffered tremendously, and I therefore was not able to take the final exam and complete the course. In the last few weeks of the summer I decided to complete a stay at Princeton House Behavioral Health which is a mental health facility that offers in-patient and outpatient treatments. I am currently un-enrolled for the fall 2020 semester and am now completing the facilities intensive 12 week outpatient program to continue to help my health. As of now I am enrolled in the winter 2021 semester and hope to return to kingston for the remainder of the year. Attached you will find a discharge sheet that summarizes my stay; including the date of admission and date of discharge to and from the facility.
