Petition Letter: Dear Associate Dean (Studies)

Good afternoon, my name is Kristen D'Angelo, my student number is 10022495. I am a fourth year Art History major here at Queen's University. I am writing this appeal to explain my current situation regarding acquiring credits that I have earned from another university institution. Due to a mix up in course codes, I was unable to acquire these credits. I am requesting permission to apply for a retroactive Letter of Permission for Summer 2013. Currently, I am in the process of finishing my degree in the Fall of 2015 and would like to use the credits from Ontario College of Art and Design (OCADU) to be transmitted onto my transcript. I have taken Studio Fine Art courses at OCADU in the Summer of 2013 and 2014. On my Letter of Permission for Summer of 2013, I had written the wrong course code. On my transcript from OCADU, the course code is DRPT 1C01, but I had written DRPT 2C03. The reason I had gotten these course codes mixed up is due to a misunderstanding in the Summer and Fall/Winter course codes on the OCADU website. When I had enrolled in March 2013, I had written on my Letter of Permission the courses I wanted to take. The course DRPT 2C03 is a Studio Painting course in Fall/Winter. In retrospect, I should have checked to make sure I was looking at the appropriate term and course code. On my OCADU transcript it is recorded I have taken DRPT 1C01 (Introduction to Painting) which is a full year course and earned 6.0 credits. In regard to this mix up, I am requesting permission to apply for a retroactive Letter of Permission for Summer 2013, so I may record the correct course code acquire these credits and complete the requirements for my BAH.

Academic Plan: Dear Associate Dean (Studies),

My Academic Plan, if my appeal is granted, is to apply for a Letter of Permission for Summer 2013 for my course taken at OCADU. I will fill out an application form, provide the Introduction to Painting course description and submit the application to Karen McBride
(Curriculum and Letters of Permission Assistant). Then, hopefully in the upcoming weeks, I receive a notification that my OCADU credits have been successfully transferred, I will be able to view them on my Queen's transcript and my BAH elective requirements will be met.
