Submit an Academic Petition or Appeal

Petition Letter: Hi Dear Art& Science Faculty Staffs and Dean.

I was in illness during entire of Fall term, and it is obviously that this case has influence my study. So, I want to apply two appeals for protecting my GPA.

According to the condition of credit standing, the course satisfies regulation are CISC 324, MATH221. And another request is to change the grade distribution of CISC 457.

My illness were found at late of July 2020, because that time was in summer break, so I went to KGH emergency department for seeking medical help. The first surgery happened in late of August. But after that surgery, the wound was not complete healed due to unknown reason. At middle of Oct 2020. My body shown side effect of that illness which the spot was in infection. The blood and pus keeping drain out of my spot and sometimes I will getting in fever(not covid-19 ). At this time, KGH said that due to covid-19, I have to wait at least one year to see the doctor, they will also sent me to other hospital. I was calling every hospital listed in the doctor note but no one could accepted my case.

The time between Oct 2020 and Dec 2020, I was trying to study my online course but my body can not supported me to get a higher grade. Side effect like fever, pain, spot drain out were almost destroy me psychologically and physically and makes me hard to focus on study. After the final exam period, I went back to the SWS(Student wellness center) again for seeking help for my illness, but the doctor's answer is same with KGH, nobody could help me but wait for contact with other hospital.

I was suffering and this illness also puts a huge stress to me. I really worry about is will develop to more serious stage of illness. By contacting with my parent, I can back to China to receive medical help. As this actually is my fifth year in Queen's as a fourth year student, my study permit extend application took about 7 month to finish( May 2020 to Oct 2020) due to covid-19, so this is anther reason I can't leave Kingston.

My documentation contain 2 doctor notes from KGH emergency department( August 2020 and Oct 2020). SWS didn't gives me anything like doctor note or other things like that, but I will still submit my appointment email as a evidence.

For the CISC 324 and Math 221, I want to apply credit standing because of my illness,I can not study well. The other appeal request is for CISC 457.

For CISC 457, I want to apply to AG. This course have 6 part and each part takes 16 marks and 100 in total. Quiz 1 and assignment 1 was happening during Sep 2020 and Oct 2020. For both of them, I didn't working well. I have told instructor in advance because I was trying to ask professor to change the grade weight of each parts, but he want me to contact art and science faculty and ask faculty to contact with him. Both mark of Quiz 1 and Assignment 1 are posted during Nov 2020 which is really late. And I don't know that is this able to appeal but I really want to pass this course. If my mark could get rid of Quiz 1 and assignment 1, I could pass this course and it is also very important for me.
