Submit an Academic Petition or Appeal

Petition Letter: I accepted my offer of admission to start in the Fall 2018 semester. However, shortly after the semester began my father became ill, and he was laid off from his job. In order to help my family financially I decided that attending school would have to wait. At that time I cancelled my OSAP application and I thought I dropped my courses, but it looks like I never did. I believe I attended one seminar though I don't even remember which course it was for. And I attended a couple of classes as well but again, I don't even remember which courses they were for.
So as far as submitting any coursework or taking tests, I never had the chance to do either. I tried to balance working full time with my schedule at first but it just was not a real option, and unfortunately I had to choose work over school.
In any review of class attendance you'll find I was not there, and had no communication with any instructors that I can recall. I'm happy to answer any more questions on the matter as best I can. I'm also not sure what documentation I would have to submit for this appeal but if there is something please let me know.
