Submit an Academic Petition or Appeal

Petition Letter: During the period the End of Term Reflection survey was due, through no fault of my own, I was unable to take medication prescribed to me to treat Major Depressive Disorder. Normally I take the medication on a daily basis, and as a result of mistakes made by my pharmacy in Kingston, I missed my daily doses for roughly a week at the end of term. As a result, I experienced withdrawal symptoms from the medication which included digestive issues, sleeping problems, and extreme mood swings. Prior to this assignment, I had completed all of the deliverables in the course on time, and had a B+ average in the course overall. Unfortunately, due to the side effects of the medication, and the stress caused from not taking it, I was unable to complete the final task for ENIN240.
I am appealing for an IN designation now and not at the end of last semester because due to my symptoms and the exams in the following weeks, I did not realize I had not completed the task until grades were published on SOLUS late last week.
Included in my appeal is a signed doctor¿¿s note testifying that I am prescribed medication for mental illness, and that I was both unable to take it at the end of term, and was experiencing side-effects.
Should my appeal be granted, I will complete the End of Term Reflection at the earliest possible opportunity, in order to resolve the matter as quickly as possible, and eliminate any ongoing obligations for the class¿¿s professors or myself.
