Petition Letter

Petition Letter: APPEAL

The student (appellant) at Queen¿¿s University named Michael Edwards hoping to complete the remaining credits to complete his BSc degree (General) in Life Sciences. Included in this application is the certificate of enrollment. This appeal enables this student to take finals and submit work that remains omitted due to the COVID-19 (2020 to 2021).

I request the following in general terms:

1. To complete courses with an incomplete grade with a final examination date.
2. To complete courses, including all the assignments, examinations, and coursework that went missing or ungraded. Previous courses that missed the finals are added to the list above.

Here is the statement of the facts about the case:

1. The final piece of work in the GLPH 385 will be assessed and added to the grade. This piece of final work, worth 30, was filed with OnQ approximately on December 31st and early January 1st 2021, due to medical and personal problems over the period (See Doctor's Note).
2. The final exams from Fall 2020 must be taken for PHAR 380 and CHEM 381 in the summer term examination slot. Although permission was granted to take the examinations, the extenuating circumstances were too difficult for me to take exams.

3. Assignments were missing and midterm exams for BCHM 270 in the Fall were missing. Although I was forced to schedule another examination date, my circumstances conflicted with this arrangement. There were numerous power outages and health problems (See Doctor's Note).

4. Examinations were missing, and midterm exams may have impinged for PHGY 215 and PHGY 216 that were missing. Although permission was granted to take the examinations, the extenuating circumstances were too difficult for me, making it possible to embark upon the scheduled exams. Dr. Ward ignored extenuating circumstance in the Fall term altogether because of his intolerant attitude towards the appellant.

5. Examinations were missing, and midterm exams may have impinged for PHAR 100 that were missing. Although permission was granted to take the examinations, the extenuating circumstances were too difficult for me to embark upon the scheduled exams.

6. Eye problem make some barriers that affect my planning, and results due to the symptoms (See Doctor's Note). In FREN 107 and I could not take Quiz 3 because I was late to find a quiz on the day, having planned to take it.

7. The summer courses' final examinations were not taken due to the medical problems experienced in the year 2020. This trend is NOT my fault and remains due to causation and discrimination in the environment (See Appendices).

Request:

I hereby request the following:

1. To include the final assessment essay submitted on the 30th December to the 1st January 2021, worth 30 in the GLPH 385 to complete the credit to avoid data loss. The previous term grade was 57/70 points without the final assessment essay. Therefore, the final grade does not represent the term work.

2. To sit all the Fall term examinations that were missing, including PHAR 370, CHEM 281, and PHAR 100, as soon as possible (with similar permission was disclosed in communications). One¿¿s term¿¿s work in each course was over 80 without the final work so, without a final, it would heavily penalise me. Therefore, the current final grade does not represent the true outcome.

3. To sit the final examination for PHGY 215 and PHGY 216 (Dr. Ward) to complete the summer term credits at the end of the summer term with access to course materials. The term¿¿s work in each course was over 80 without the final work included. Therefore, the final grade does not represent the outcome of the term¿¿s work earned by the appellant.

4. To take the midterms for BCHM 270 and include the missing pieces of work similar to the winter term for both credit hours course offerings (Winter, Fall). Therefore, the final grade does not represent the real outcome of the term¿¿s work.

5. To take the missing quizzes in each of my courses in the academic year 2021 and 2020. Therefore, this remedy will prevent misrepresentation of my class rank from happening again due to missing marks, which lower the real scores to create outliers and failures.

6. To provide the coaching, missing teaching materials, tutorials, live sessions and exams.

7. To remove all academic penalties placed on the appellant's accounts due to extenuating circumstances and bias from the academic calendar's start.

8. To have a chance to obtain a subsidy to get a decent set of prescription eyeglasses before I take my examinations.

9. To have financial support, compensation, and reprimands placed on the culprits for one¿¿s academic studies and for negatively affecting my studies on previous occurrences of discrimination, bias, and administrative decision making (See Appendices).

Academic Plan

Academic Plan: I have received approvals for these courses recently or in the past therefore, I suggest the following:

I will take the PHAR 380, BCHM 218, and CHEM 381; May and June 2021;

I will take the PHGY 218, PHAR 100, PHGY 216 in and August 2021;

No summer registrations are foreseen.

I will take BCHM 270 or BCHM 218 in July 2021 (Summer).

I would be allowed to follow the courses online with OnQ;

I have received permission to sit PHAR 380 and BCHM 218.

New Information: Dear Dean,

I would like to ask permission to take my examinations at a convenient time. At present, I have five examinations in one week in the deferral period. I have eye vision problems with diabetes and this combination causes fatigue and migraines. Can some of these exams be taken over the summer

Also, I had some illness problems in the past related to diabetes and a virus. I also took a 2-year hiatus. So, I had to take courses a number of times because I had missed an exam or assignment. I would like the entries for these occasions blotted out or Agregat (AG) because my estimated grade was usually an A or B.

Yours sincerely

Appeal Letter

Appeal Letter: Dear Dean,

I have had some challenges with my health this year which were not entirely expected. My vision problems and COVID-19 type of symptoms began somewhere between the Summer Term 2020. and the Winter term 2021.

In the summer of last year (2020), there were symptoms of vision strain and irritation with bowel pain. Visits to the Doctor proved there were no legions in the bowel. However, there was eye haemorrhaging in the right eye. I visited various doctors to check my body and they were not sure about the diagnosis. As a result, it was difficult to concentrate on the text during the first year. Thus, I enclose the medical certificates for certain relevant periods of time. Also, when starting studies there were vision problems and bowel discomfort for approximately 6 months while studying at university with a combined effect.

Unfortunately, the provinces of Canada were in lockdown due to the COVID-19 pandemic which meant it was not possible to obtain a medical report immediately or on the day of the exam.

Over Christmas 2020 and Easter 2021, I was off-sick during a period where one usually prepares key assignments and examinations. Although I hoped to take the deferred examinations, I was not able to do this with continuing vision problems and some environmental intrusions at home on a daily basis.

I would like the opportunity to take the examinations that were suspended in a cycle of not attending examinations or omitted coursework in one particular case. I believe that I completed at least 60% of the coursework in the incomplete courses.

There seems to be a discrepancy with the information provided by the Dean in the last appeal. I know that I have completed the learning objectives and the coursework with midterms. Therefore, my secondary request is that my courses, where appropriate, are averaged to give the course grade for the Summer Term and other courses that required only the final.

The courses are PHAR 100, PHAR 370, PHGY 215, and PHGY 216.

However, I found the course BCHM 270 is particularly difficult to complete under the circumstances of my vision and health problems. Therefore, I would like to opportunity to complete the final and/or missing parts at a later date.

Finally, the GLPH 385 was missing a final essay in the course assessment. The instructor informed me by email that she would have accepted the essay if she had a doctor's note. I was unable to provide a note because my doctor's office was not open to the public through certain weeks of the pandemic including December 2021. I did obtain a note for the December period after January 2021 which I showed her and she said this could have been admissible had it arrived sooner. Thus, I request that my essay be included in my final grade for this credit also.

I have agreed to sit my final exam for CHEM 281 and accept the average mark for MICR 270 with the instructors.

Yours sincerely,

Michael Edwards
