Submit an Academic Petition or Appeal

Petition Letter: To whom it may concern,
I am hoping that my exam grade could be redistributed to my course work and assignments.
Firstly, I was struggling with my mental health especially towards the month of July because of my busy work schedule that I needed to maintain in order to afford rent and schooling and the heavy demands of the course. Around the last week of July my grandfather started getting really sick and we made frequent trips to visit him and found out soon after that he has stage 4 cancer. My mom decided she needed to live with him in order to care for him, which has been really a really difficult and stressful time for us all. Unfortunately because of this my mental health declined even more than it had already been and I started and continue to feel extremely overwhelmed and exhausted. I was not at all in the right headspace to go through exams and postponing it would not have made much of a difference since these unfortunate events are ongoing. I knew immediately coming out of the exam that I was not successful and followed that up with an email. I was just not focused on the content and most of my knowledge slipped my mind. I have also been suffering from anxiety, depression and epilepsy. I feel extremely disappointed with myself but I know it was out of my control. I discussed the possibility redistributing my exam grade into course work, with my professor before the exam, and was hoping that this is still possible. If not, I would request if it is possible for me to receive a NG in the course, or make up some assignments to earn a passing grade. Thank you.
