Petition Letter: I am writing this letter to appeal for the extension of the deadline to submit incomplete work in a class.
The two main extenuating circumstances that affected my academic abilities included developing severe depression/anxiety and needing to take care of a sick family member. Earlier this year when I first started the course, I started working at a laboratory that dealt with covid samples only because despite my efforts I was unable to find a non covid related job to support my family. I can supply my letter of employment showing this. I developed severe depression and anxiety that was pandemic/COVID related as my family and I are apart of the susceptible immunocompromised group. The depression and anxiety has lasted all year and affected my studies and daily life. I was unable to do my school work and exams because I was going through the following symptoms from my mental illness: lack of energy, excessive sleeping, panic attacks, being unable to concentrate in general, mood swings, suicidal thoughts. Throughout this time, I was disconnected from support from friends and family due to the pandemic. I regularly followed up with my doctor and even tried taking depression medication which gave me unwanted side effects. I will be attaching my doctors letters, prescription, and emails to my professor. After I received an extension into the next semester still struggling with depression and anxiety, my mom had been scheduled for a surgery. I spent two months taking care of her as she was unable to perform any task herself. I was unable to put my concentration into school at this time as well. I can provide a letter from her surgeon with proof of date and surgery.

Academic Plan
Academic Plan: My plan is to continue following up with my doctor and finding medication that can better help me cope with my symptoms from the mental illness. I know that my mental illness may be long term and range in severity so I will ensure to rethink my decision for applying to take a course and apply for academic accommodation before the course starts. My levels of anxiety have been lowered as I am finally being able to take the vaccine (doctor was investigating my autoimmune issues) and I feel much safer. I have also recently been offered and have started working from home for the same company I started at in the year which is also making me feel safer and less anxious. I am also planning to get in contact with advisors/councillors from Queens University to get advice and help for my mental health issues. I believe if I finish this course, I can prove to myself that I am capable of solving my issues and it will contribute to helping me heal my mental health, along with all of the other steps I have listed. I feel like my mental health is already improving and id like another chance to complete this course. Thank you for considering my appeal.
