To whom it may concern,
My name is Danica Wall, I am appealing to extend a grade designation of IN for CHEM497 in Fall/Winter 2020/2021. Due to a flare up of a mental health disorder I was unable to complete the course work as scheduled. I struggled a lot during the year with the pandemic and a traumatic event that occurred October 31st that I was not able to complete a full academic course load. I submitted a short-term academic consideration form for November 2nd and February 3rd, 2021, and later in the semester registered for QSAS.
I am currently planning on finishing the deliverables for this course during Fall 2021. If any further documentation is needed, please do not hesitate to contact me so that I can provide any additional documentation that would be required for a successful appeal, such as a doctor??s note or note from my counsellor. I thank you in advance for your consideration.
