Submit an Academic Petition or Appeal

Petition Letter: To Whom it May Concern,

My name is Jadyn Clark and I am a second year student in Health Sciences. I would like to request an appeal for a CR mark for my PHYS 118 grade from the Summer 2021 term.
I do not feel this grade reflects myself as a student, as I always demonstrate academic excellence and excel in my studies, but this mark does not.

Throughout this time, I struggled greatly with depression and anxiety. Specifically the last couple weeks of the course and during the final exam period. I often felt unmotivated, could not concentrate, and could not sleep.

To combat my struggles in the course, I contacted a tutor. This was very helpful for homework assignments, however, I still struggled greatly.

I also consulted my doctor, who doubled my dosage of medications, increasing me from 75 mg of sertraline daily to 150 mg daily. She also prescribed 25 mg of quetiapine nightly to help me sleep. It took a couple of months for my condition to improve, with me noticing an improvement in myself late August.

As an aspiring medical student, I cannot afford for this mark to have a damaging effect on my GPA and chances of getting into medical school.
