Petition Letter: Dear Sir/Madam,

RE: Stacey Marks, student number 20142971
PSYC 271 (Summer 2021) deferred final exam

I hereby submit a request for an extension of my incomplete grade and approval to defer the final exam, which expires on December 31, 2021. The circumstances relating to my request are as follows:

I am currently experiencing some health related issues. For the past few months, I have been dealing with illnesses relating to my medical conditions. My sicknesses have caused my health to deteriorate significantly. I am presently not in a position to complete my upcoming deferred final exam.

I have recently started a new course of treatments. However, I am in the early days of treatment, and based on my treatment plan, it will take a few weeks for my health to improve. I am requesting your consideration for approval to a further extension to write my exam during the next deferred exams period.

Please be advised that I am currently registered with QSAS should you require any supporting documentation relating to my medical conditions.

I also wish to thank you in advance for your kind accommodation.
