Petition Letter: My name is Kaiyuan Guo, and my student number is 20133685. I am currently studying on the Life Sciences program. Throughout this term, I have experienced a series of extenuating circumstances which have led me to miss the academic deadline for dropping the BCHM315.

I was diagnosed with thyroid nodules in September this year, and my physician recommended that I be retested in three months. From September to December, I often felt weak and mentally depressed. Since November, I have noticed that my neck has become thicker and swollen around the lymph nodes, which began to worry me once more. During a recent examination, I was found to have diffuse lesions in my thyroid gland, which does not exclude the possibility of malignant lesions. Combined with the problem of my swollen lymph nodes, my physicians suspect that I have developed immune system and endocrine system disorders. In this regard, I need to undergo further examinations.

As a result of this, my mental state was terrible. I often felt dizzy and had pain in my neck. I also experienced tachycardia and had difficulty breathing. I was thought to be suffering from autonomic dysfunction. In addition, various neurological symptoms have been bothering me, such as chest tightness, increased heart rate, tinnitus, dizziness, and headaches. As a result of this, I started taking medication. However, the medication also caused the side effect of vomiting. I have been trying to persevere to finish the whole semester, but my physical and mental condition is not enough to support me to continue doing so, which I why I am applying to drop this course.

The constant hospital visits and examinations, alongside my lack of energy caused by my low mental state, meant that I had little time to submit the application to drop this course prior to the academic deadline. I sincerely hope that you will make an exception for me in light of the circumstances I have suffered through over these past few months.

New Information: I am an international student, and I am now staying in China. Due to the epidemic, I have been taking courses remotely in China from the fall 2020 semester until now. Since I am in China, the physician who provided me with medical care is a Chinese doctor.

Therefore, I asked the Chinese doctor to fill out the off-campus-physician-note and translate them into English. The documents contain the Chinese version completed by the physician and the translated English version.

In addition, the doctor who provided my medical services is a Chinese doctor, so he does not have a CPSO. Thus, the Registration No. CPSO item in the form was not filled in.
