My name is Angela Chan, and I am a fourth year Queen¿¿s student majoring in Biology. I am writing you to appeal:
1) For Credit standing in Dr. Eckert¿¿s BIOL 315 (Win, 2014),
2) To drop a failed course in Dr. Carran and Dr. Zechel¿¿s CHEM 281 (Fall, 2014),
3) To drop the following courses after the academic deadline: ENSC 320 (Win, 2015), ENSC 301 (Win, 2015), ENSC 201 (Win, 2015), and GEOL 106 (Win, 2015).
In 2009, I was hospitalized and diagnosed with anorexia nervosa and anxiety disorder. Before making a full recovery, I went against my doctor¿¿s wishes and started post-secondary at Queen¿¿s (Fall, 2011). After a traumatizing attempt at seeking professional help from La Salle on campus (Fall, 2012), I relied solely on anti-depressants; but it had little progress with my condition. I continued to suffer major depression, anxiety, and adult attention deficit disorder, however, was too scared and guarded to seek further help. In October 2014, my condition became very severe: I struggled to leave my bed, I failed to comprehend material and attend lectures, I was late to examinations, I missed deadlines, assignments, etc. In mid-January 2015, I had a severe mental breakdown, and stopped attending classes entirely. By February, my condition triggered numerous episodes of grand mal seizures, which later lead to hospitalizations and medical testing. In March, I spoke with an academic adviser and we decided it was best to drop all my courses.
During this period, I was not mentally, emotionally, or physically fit to perform to my fullest potential, and my academic performance was greatly affected. I earned uncharacteristically poor grades, and failed to effectively confront my academic responsibilities in an appropriate and timely manner.
Alongside my deteriorated psychological health, the frequent occurrences of my epileptic episodes left me in an exhausted and sedentary state. With all the scans, tests, and probing from various doctors and nurses, I became even more overwhelmed and depressed, and completely ignored my academic responsibilities. I acted thoughtlessly and I take full responsibility for my reckless behaviour.
In mid-March, with the support of my parents, I finally sought professional help and accepted the idea of counselling. While I have been wait-listed for psychiatric help in my home town, my neurologist has helped me make arrangements to see a psychologist in late-April. In the meantime, I have started attending weekly one-on-one sessions with the local Mental Health Urgent Response team, and my general practitioner has increased my medication dosage. While my recovery process is just beginning, I believe I can make a full recovery. In one years time, I plan to happily return to Queen¿¿s, where I am able to perform competently with a reduced course. Upon returning to Queen¿¿s, I intend on establishing strong lines of communication with my family, doctors, and support team in order to maintain regular contact. Moreover, I am confident that after this experience, I will develop better coping strategies and management plans, which will help me confront future distresses in an appropriate and effective manner.
While I continue my rehabilitation, I hope you will grant my appeals for dropping four courses after the academic deadline, dropping a failed course (CHEM 281), and a credit standing in BIOL 315. Attached, you can find two letters from my doctors. Please let me know if I am missing any information, or failed to provide a sufficient amount of documentation.
