Submit an Academic Petition or Appeal

New Information: Hello there,

I am uploading a screenshot from an email I received from QSAS after they received documentation from Student Wellness Services supporting the impact my extenuating circumstance had on my academics. That documentation was provided by my counsellor at the time, Solita Hoogendam.

However, I do not have that specific documentation myself. I have been emailing several people to try and retrieve it but have yet to receive responses or support.

I do not want to miss Academic Appeal deadlines, so I am uploading the information I have at the moment.
