Petition Letter: To Whom It May Concern,

The aim of this appeal letter is to explain my extenuating circumstances which have affected my ability to perform to my academic potential on two courses: 316 Anatomy and 255 KNPE. Consequently, I am requesting credit standing on 316 Anatomy and an extension to write the deferred exam for 255 KNPE.

Since mid-March 2022, I had to visit the Kingston General Hospital Emergency Department on three separate occasions for a physical illness that was treatment resistant necessitating the need for aggressive antibiotics and steroids. Symptoms included: fever, nausea, gastrointestinal issues, inability to swallow, extreme fatigue and unable to get out of bed to perform daily tasks such as eating and studying. This resulted in challenges to prepare and study for the 316 Anatomy and 255 KNPE assessments.

Specifically, due to these extenuating circumstances I had to keep deferring my 316 Anatomy assessments: test 3, final lab exam, and final exam. I felt ill prepared as dealing with my mental and physical health took over my abilities to attend class and study. KNPE 255 had two midterms for which I was not able to study.

The physical illness exasperated an existing diagnosed mental health issue in that I could not swallow or keep down my prescribed medication. This vicious cycle remains ongoing and I am in the process of getting help out of this dark time. Currently, I am in the process of being referred to a specialist to address the physical illness. Also, I have an urgent appointment with my family physician on 17 May 2022, a second appointment with my psychologist on 18 May 2022 and an appointment with an academic advisor on 18 May 2022.

This situation is very disappointing since I know I can do better academically. For example, last year I did not have any physical illnesses beyond my control and my mental health was adequately regulated and stable. This allowed me to successfully complete all of my course requirements.

In summary, the second half of this semester has been negatively impacted by an ongoing physical illness, which has also affected my mental health. These extenuating circumstances beyond my control had a major impact on these two courses. Consequently, I am asking for credit standing on 316 Anatomy and an extension to the deferred exam for 255 KNPE.
