Petition Letter: To Whom It May Concern,
My name is Hannah Harilaid, and I wish to apply to waive my requirement to withdraw.
This past year has been tough for me, with my disability and reoccurring mental health struggles:
I struggle with severe mental illness (depression, anxiety, OCD, and PTSD) which is documented with my QSAS advisor, Angel Varette.
In December of 2021, my doctor changed the medication I take, which caused me to become severely disassociated and unable to focus for around 2 months - I have since adjusted to the change and do not face this issue. I then got very sick over the spring break which prompted a two-week breakdown where I was convinced I was inflicted with Covid-19 and was going to infect those around me. During this time I was unable to write midterm exams, or catch up on the work I was behind on, I had planned to work through my missing assignments during this time but was unable to. In March I emailed Ms. Varette and we discussed her reaching out to my professors about extensions on my assignments or requesting my classes be marked as incomplete until I could complete the work. I have not heard back from Ms. Varette as to my teacher's responses. In addition to all of this, I recently turned 18, and have been dealing with many legal issues with my family in regard to my late grandfather's estate. I have also not been with my support animal for a majority of the year due to issues with paperwork, the application took too long and it would be unfair to her to move her for such a short period of time.
I am aware I am responsible for my own disability, and I am very sorry and ashamed to be writing this. I am generally not a bad student but this has been a very hard year; looking at my marks for my completed course, you can see I achieved an A-.
If there is any way I can retake the courses next year, finish the work over the summer, drop the courses late, or be placed on academic probation, I would be forever grateful.
Thank You for hearing my appeal. If there is anything you are unclear about or would like more information regarding, please contact me.
I hope you have a good day.

Academic Plan
Academic Plan: I intend to make substantial efforts to improve my academic standing:
I will commit to seeing a therapist or psychiatrist frequently and consistently. In the past, I have worked with therapists but I have not seen one since the summer of 2021.
I will take advantage of the support systems and resources Queen's offers, attend SASS workshops, as well as apply for the Peer Mentor Program.
I will make an effort to reach out to a therapist or to the school if I am struggling, instead of waiting until it is too late.
I will designate a space and time every day to work and work on a weekly schedule, as opposed to when I think I can work. When I was staying in residence, I could not focus enough to work in my room and, because of the mask mandate, I found it very distracting to study in public spaces (I wear glasses and they get very foggy with masks, especially during the winter)
I will bring my support animal to Kingston to stay with me during the school year.
I will initiate or join a study group in classes I anticipate struggling with.
This year has served as a wake-up call and I now realize I need to work with my disability instead of against it.
