Petition Letter: I am appealing for a waiver for the requirement to withdraw from the Faculty of Arts and Science for one year. My extenuating circumstances include physical illnesses and psychological challenges.
The direct connection between the extenuating circumstances and their effect on my academic performance is that they have caused me to not to be able to concentrate effectively on my academic studies. The events occurred around the month of October in the year of 2021, affected me constantly and consistently and is ongoing, all my works in all my courses were affected. I have not taken any step to deal with the extenuating circumstances during their occurrence as I was hardly aware, I recently however had a consultation with a health care professional at the Côté Sharp Student Wellness Centre of the Student Wellness Services.

Academic Plan
Academic Plan: An academic plan has not been generated as the discovery of many of the medical issues are quite recent, and the severity of them is not clear. I so would like a leave of absence instead of a required withdrawal and hopefully generate an academic plan during my leave.
