Dear Associate Dean (Academic),

My name is Desiree Moses. I am a student registered in the School of Nursing taking PSYC100 during summer 2022 term. I am writing this letter to request and appeal to retake the midterm examination for PSYC100 ?? Principles of Psychology. I am a student registered with QSAS, and I wrote the exam with accommodations, but due to my condition I had fallen a sleep during my exam which caused me to perform poorly. I was incredibly tired during the exam and this affected my ability to perform at my full potential. During my exam, I struggled to keep focused and keep awake. This issue has been the topic of discussion at many Doctor??s appointments regarding my condition. I have an ongoing condition that causes symptoms that affect my everyday life and unfortunately one of those symptoms affect my performance during the midterm examination. I am currently taking medication to manage my condition, but finding a compatible medication is an experiment of its own. My doctor has increased my dosage and We are hoping that this will help.

Unfortunately, I submitted the exam knowing I did not perform well because of inability to focus and concentrate and if given the opportunity, I can perform better. I can be successful in this course but the midterm is worth a large portion of the final grade (30%), so in order to be successful in the course I must do well on the midterm. I know that I can do better, if granted the opportunity. I look forward to receiving your response.
