To the Associate Dean (Academic)
Queen's University

Dear Associate Dean (Academic);

The Request: I am petitioning to drop MATH 121 PHYS 115 (Fall, 2022) for medical reasons.

Explanation of Extenuating Circumstances: I have special needs, already documented in my file. Myself and my doctor, Dr. D.W. Richardson, was expecting that I would have been able to complete my full-time, 5 courses load, necessary for admission in the Doctorate of Veterinary Medicine program thereafter my degree in Biology.

My extenuating circumstance results in requiring more time to process the information that I am given. I know I am able to understand all the material of the 5 courses. However, I was not able in the timeframe allocated with the 5 courses at the same time. Since October, I can clearly see that I am experiencing a tremendous level of difficulty to recover and follow the course of action necessary to succeed in all my courses. In order to obtain the best grades possible, I had prioritized my 3 other courses. This is why they are not as severely affected as the ones I am currently petitioning to drop.

On November 6th, I had an evaluation performed by my clinical psychologist, Alice Rushing. My doctor, on November 25th, wrote a letter recommending my course load reduction. On December 1st, I consulted with Alice Rushing during a follow-up appointment.

I am making my request at this point in time only because I did not know it was possible to do such a request, until a medical professional explained it to me. As soon, I was made aware, I started the process necessary and consulted with my clinical psychologist and doctor.

Future plan: It is my clear intention to succeed and complete my degree in Biology at Queen's University. I am currently receiving great services from Queen's Student Accessibility Services (QSAS), who directed me even for the present request. I also am attempting to get an appointment with an Academic Advisor. I want to meet with an Academic Advisor to adapt and take a lighter course load in the future.

I will continue to get access to the assistance available for students with special needs from QSAS. I will maintain regular meetings with my doctor to continue to receive the professional medical support I need.
I will withdraw for my course MATH 121 for this semester, to help reduce the course load and give me the chance to succeed.

Finally, I will soon receive a report from my clinical psychologist. This report will give me insights and guidance in the best approach to achieve my academic goals.

Sincerely,

Jean-Frederick Tetreault
