Submit an Academic Petition or Appeal

Petition Letter: Dear Associate Dean (Academic),

Please consider this first level petition for complete academic withdrawal from studies at Queen's University and for late course drop for HIST105 (2022).

I am submitting this petition for extenuating circumstances due to medical reasons.

I started Queen's University with optimism and enjoyed my classes at the beginning of term, especially Sociology. However, I became ill in mid-September and had a series of subsequent related illnesses that had a negative impact on my academic performance and academic success at Queen's University. Since mid-September, I have been under the care of Dr. Robert Neville, my family physician. A letter from Dr. Neville is submitted, providing evidence for this petition.

Due to ongoing illness, I returned to home to Peterborough the third week of November. From this date I no longer attended classes or submitted assignments.

I will not be returning to Queen's in January 2023. I vacated my room of belongings on December 12. Yesterday I returned my room keys and student ID card via courier, separately to the specified addresses and both of these have been confirmed as delivered by the courier.

I am a good student with good work habits. When my health permits, I plan to pursue a post-secondary education. For this reason, it is important to me that my academic record at Queen's does not prejudice future post-secondary education.

Through this petition for academic withdrawal from studies at Queen's University and late course drop without academic penalty I want to ensure that my transcript reflects academic withdrawal without academic penalty, showing that courses were either dropped or that no grade was assigned.
