Appeal Number: 1333
Decision Date: November 27, 2015

Student Email: 13agc1@queensu.ca

Dear Ms. Chen:

I have carefully considered your request to have your appeal heard after the 21-day timeline for submission, and have decided, based on the circumstances you refer to in your letter of appeal, to waive the timeline. You may now proceed with your planned appeal to waive the requirement to withdraw for one year.

Please access on the on-line appeals system at: http://www.queensu.ca/artsci/help/academic-appeals/online-appeal-submission After you enter your NetID and password you will now have a new appeal option called €œSubmit a Late Appeal€. Choose this option and follow the steps. Your appeal letter should explain the extenuating circumstances concerning your academic performance last year and you should include some medical documentation to confirm the full timeline of your condition, it€™s severity and the impact that it had on your studies last year. It would also be helpful for your doctor to confirm whether or not you are currently recovered enough to make a successful return to your studies.

There will not be a fee for this new appeal, as it will be considered an amendment to your 21-day deadline appeal so the system will skip over the payment step.

If you have any questions, please feel free to contact the Faculty Office for advice or assistance.

Sincerely,

Johanne Bénard, Ph.D.
