You are permitted up to the final day of the December exam period (December 21, 2016) at the latest to write this exam but I would suggest that you write it as soon as possible. It is your responsibility to contact Professor Koeppl in a timely manner to make the appropriate arrangements and confirm the time and date of the exam. I would suggest that you forward him a copy of your documentation as well.
