I have carefully considered your request to have your appeal heard after the 21-day or one term deadline for submission, and have decided, based on the circumstances you refer to in your letter of appeal, to waive the timeline. You may now proceed with your planned appeal to address courses from the Fall term 2013 and Fall term 2016.

Please access on the on-line appeals system at: http://www.queensu.ca/artsci/help/academic-appeals/online-appeal-submission After you enter your NetID and password you will now have a new appeal option called €œSubmit a Late Appeal€. Choose this option and follow the steps. Your appeal letter should explain the extenuating circumstances concerning these particular courses, and include any relevant supporting documentation that wasn€™t included with your prior appeal. There will not be a fee for this new appeal, as it will be considered an amendment to your 21-day or one term deadline appeal so the system will skip over the payment step.

An important part of a successful appeal is documentation which address the severity, impact and timeline of the circumstances presented in your appeal letter. I am including a link to our Off-Campus Physicians form which will be beneficial as you provide documentation for the 2013 time period as well as the 2016 time period.

You will also want to address in your appeal letter an explanation as to why MATH 231 (F 2016) was impacted by your extenuating circumstances more so than the other courses you were registered in that term.
