Dear Mr. Bopp:

I have carefully considered your request to have your appeal heard after the 21-day timeline for submission, and have decided to waive the timeline, based on the length of time it took to seek help and obtain documentation. You may now proceed with your planned appeal to waive the requirement to withdraw for one year.

Please access on the on-line appeals system at: http://www.queensu.ca/artsci/help/academic-appeals/online-appeal-submission After you enter your NetID and password you will now have a new appeal option called €œSubmit a Late Appeal€. Choose this option and follow the steps. Your appeal letter should explain the extenuating circumstances that affected your classes last year, and include some supporting documentation from your healthcare provider that confirms the timeline and severity of your circumstances. We also require confirmation from your healthcare provider as to whether or not your circumstances are now being managed or are resolved to the point where you can make a successful return to studies this fall.

There will not be a fee for this new appeal, as it will be considered an amendment to your 21-day deadline appeal so the system will skip over the payment step.
