Dear Mr. Sattari Khavas:

I have carefully considered your appeal to drop HLTH 101 and receive Credit standing FREN 106 (F 2017) past the academic deadline. After reviewing your file I have decided to grant your request.

This decision is based on the fact that you have documentation of an illness occurring at or around the deadline to drop courses for which you required a minimum of 2 weeks away from school.

The medical certificate from The Ottawa Hospital confirms that you were unable to attend school from November 2 to November 16, 2017.

On another note, please be aware that the self-declared request for accommodation up to 48 hours is not to be used repeatedly for lengthy illnesses like mono or for circumstances within your control such as family events. For the former, a request for longer term accommodation (up to 3 months) is required accompanied by medical documentation. The latter is not a bona fide reason for academic consideration.
