Decision Letter

Appeal Number: 1268
Decision Date: August 26, 2015
Associate Dean: Sue Blake
Student Name: Devon Lio
Student Email: 12dml7@queensu.ca

Dear Mr. Lio:

I have carefully considered your request to have your appeal heard after the 21-day timeline for submission, and have decided, based on the circumstances you refer to in your letter of appeal, to waive the timeline. You may now proceed with your planned appeal to request to waive the requirement to withdraw for one year.

Please note that it is important that you provide some documentation from a healthcare professional that confirms the timeline of your extenuating circumstances as well as how severe they were and the impact that they had on your academic performance over the past two years while at Queen€™s. The letter supplied by Dr. Mitges addresses your improved chance of success given your current treatment regime but does not include these additional details.

Please access on the on-line appeals system at: http://www.queensu.ca/artsci/help/academic-appeals/online-appeal-submission After you enter your NetID and password you will now have a new appeal option called €œSubmit a Late Appeal€. Choose this option and follow the steps. Your appeal letter should explain the extenuating circumstances concerning these particular courses, and include any relevant supporting documentation that wasn€™t included with your prior appeal. There will not be a fee for this new appeal, as it will be considered an amendment to your 21-day deadline appeal so the system will skip over the payment step.

If you have any questions, please feel free to contact the Faculty Office for advice or assistance.

Sincerely,

Sue Blake
Assistant Dean (Studies)
