I have granted this request due to the timeline and severity of extenuating circumstances as confirmed within the documentation you supplied. Your grades in these courses will be replaced with a DR notation on your transcript.

Going forward, I would suggest that you remain connected with Queen’s Student Accessibility Services (QSAS) each term so that you obtain ongoing support from an Accessibility Advisor who can help manage the ongoing impacts of your health on your academics.
