Petition Letter
To Whom it may concern,
I am requesting to appeal to late drop FILM 394 from Fall 202 1
I was enrolled in FILM 394 in the Fall of 2021 but I had an extension until the end of the
2022 Winter term to complete the course as I had been assigned an IN grade due to my mental and
physical health circumstances During the Winter 2022 term though my situation worsened and I
ended up going on medical leave, which meant that the IN grade of FILM 394 defaulted to an F,
since I had to interrupt the academic term once the medical leave started and could not complete the outstanding work for the IN.
I have been dealing with significant extenuating circumstances in the last couple of years,
which have impacted my daily functioning and overall well-being. This has included significant
issues with panic, depression, and sleep which have limited my daily living activities. Many of the
circumstances surrounding this period are quite sensitive and personal and my therapist has advised
that I don??t need to disclose details, but I have attached here supporting documentation from them.
I am currently taking a lower course load and am working with counseling and wellness
services to identify specific therapy methods to navigate my situation, as well as with QSAS around
accommodations and academic strategies for managing my workload.
I am almost at the end of my program, and I am focusing on completing my last courses I am
grateful for the support I have been able to access to help me navigate this period of my life, I hope
you can consider this appeal request,
